One of the essential but often neglected elements of an RCA (Root Cause Analysis), FMEA (Failure Mode Effects Analysis), and ACA (Apparent Cause Analysis) is the development and implementation of effective, realistic, strong action plans. With too many cases, the lessons learned and action plans are not shared or implemented across the organization, resulting in silos of knowledge where disseminated preventive actions could have improved safety.
This webinar will be an in-depth discussion of the critical value of relevant, strong, actionable plans after an RCA, FMEA, and ACA. Action plans are often underutilized with inadequate monitoring and follow-through to measure their outcomes and modify as necessary. This is important to prevent similar incidents in another department, to demonstrate return on the investment of human capital and resources participating in the RCA, FMEA, and ACA, to avoid other departments “reinventing the wheel” for a same or similar incident, and to empower staff to be part of the solution.
Attendees will be able to create more sophisticated, thorough, and measurable action plans and lead their organization to improved implementation and spread of the action plan.
Learning Objectives:
- Establish why and how to develop strong action plans after an RCA, ACA, and FMEA
- Describe how to monitor action plans for sustainability
- Discuss strategies and ways to spread the lessons learned and resultant action plans to break down silos and improve safety across the organization
