Effective communication is a cornerstone of patient safety, particularly during transitions of care, when critical information must be accurately exchanged between health care providers. This webinar will delve into the risks associated with ineffective handoff communication and offer actionable strategies for mitigating these risks.

Participants will learn about the significant impact that poor information exchange can have on patient outcomes, as well as best practices for improving handoff communication to ensure continuity of care. The course will also explore strategies for gaining senior leadership support to implement a standardized framework for handoff communications, fostering consistency, and enhancing patient safety across the organization.

By the end of this webinar, you will be equipped with the knowledge and tools to reduce risks associated with communication breakdowns and champion more effective handoff processes within your organization.

Speaker:

Robin Lynch, MSN, RN, CHSE, CPPS,  is the Manager, Quality, Risk & Patient Safety at Med-IQ. Robin holds certifications in simulation education and patient safety. She has presented at national and regional risk, patient safety, and simulation conferences and developed custom client content on topics including clinical debriefing, care transitions, workplace violence, diagnostic accuracy, transgender health care, curriculum design and change management and served as contributing author of ASHRM's Failure Mode & Effects Analysis (FMEA) Playbook. Most recently, she designed a hybrid workshop on facilitating effective root cause analysis from a human factors lens.

Christina Hazekamp, MPH,  is a Vice President at the I-PASS Institute. In her 8 years with the Institute, Christina has overseen the roll-out of I-PASS at over 80 hospitals nationwide. She has streamlined implementation processes by incorporating best practices and principles from Lean Six Sigma and behavior change management. Her motivation to improve the delivery of care and patient safety started during her role as a clinical dietetic technician and continued through her graduate studies as she evaluated the root causes of increased costs associated with parenteral nutrition in chronically ill patients, ultimately providing her a holistic perspective from which to approach workflow and communication improvements in health care.

Georganna Horell, MHS, BS, RN, CPHRM,  is an experienced risk professional with operational expertise as a former director at a large community hospital of the Risk and Patient Safety, Patient Experience, Quality, and Infection Control Departments. Georganna currently serves as a content writer of physician and APP education specific to risk and patient safety.

 

 

 

 

Theresa Murray, MSN, BSN, CPPS, LSSBB,  as Vice President of Clinical Strategy and Transformation, coordinates the clinical coach program and oversees recruitment, onboarding, and ongoing development for I-PASS implementation programs. Theresa graduated from the University of Vermont with a Bachelor of Science in Nursing and later received her Master of Science in Nursing Education from Norwich University. Prior to joining I-PASS, Theresa worked with Dartmouth Health for over 12 years, where she held various positions and led initiatives in patient safety, process improvement, and nursing education. Theresa is a Lean Six Sigma Black Belt (LSSBB) and a Certified Professional in Patient Safety (CPPS).
