The health care industry is rapidly transitioning toward value-based care models, emphasizing better outcomes, cost savings, and improved patient experiences, especially for senior populations. In this comprehensive webinar, "Senior Living and Post-Acute Care Destinations: Aligning Value-Based Care Reimbursement Models and Quality of Care for Seniors and Healthcare Providers," we will explore how health care providers and senior living organizations can navigate this shift successfully.
This session will provide a thorough overview of value-based care principles and primary care models that are designed to reduce low-acuity admissions and prevent unnecessary readmissions for seniors. By examining key care models, we will show how they can help mitigate risk for hospitals, health systems, and senior care providers, while improving quality of care.
Additionally, we will highlight critical data and case studies demonstrating the effectiveness of innovative care models, such as Integrated Medicare Special Needs Plans (IeSNP), Long-Term Care Accountable Care Organizations (ACO), and emerging CMMI programs. These models are improving care coordination and offering significant benefits for the overall health care ecosystem.
Learning Objectives:
- Outline the principles of value-based care and primary care models that drive fewer lower-level acuity admissions and reduced readmissions for seniors.
- Identify care models that reduce care risk for hospitals, health systems and seniors.
- Highlight data that demonstrates the effectiveness of IeSNP, Long term care accountable care organizations and emerging CMMI programs the benefits of care coordination for the overall health care industry.
